 Ok, I've enhanced the show_msg_links option so that it can show links only at the top (if set to 3) or only at the bottom (if set to 4).  I've also added a show_index_links option to control the availability of the links to the index files.

 I've also added a txtsuffix option which enables the generation of individual files in mbox format (mainly for lists that have a lot of patches or source code that people will want to get in non-html format).
